Making Subject Access Requests
To make a subject access request please print out the attached form and return to the surgery in person. Otherwise you can complete the form and return it by email to sxicb-esx.themeads@nhs.net. Should you email the form it will be necessary for the necessary ID to be supplied before the information is released.
