Patients who take specific repeat medications or who have chronic conditions need to have blood tests to monitor their condition and/or an annual review.
Chronic Conditions include:
Asthma, Chronic Obstructive Pulmonary Disease, Chronic Kidney Disease, Coronary Heart Disease, Dementia, Diabetes, Epilepsy, Heart Failure, Hypertension, Mental Health, Peripheral Arterial Disease, stroke or TIA

The practice has a recall system whereby all patients who require an annual review are asked to make an appointment around the month of their birthday.
You will be invited to make an appointment in a variety of different ways:
- By Letter
- By message on your repeat medication counterfoil
- By text message (if you have a mobile phone and have consented for messages to be sent)
- Verbally
You do not have to wait to be invited to make an appointment; if it is your birthday month and you know you are due an annual review please speak with a receptionist who will be able to book you in with the appropriate Clinician and ensure any blood tests due prior to your review are also done.
All annual review appointments can be booked into the next available routine slot. If telephoning to make an appointment please ring after 10.00am.
